Notice of Privacy Practices

Our Commitment

Restorative Healthcare is required by law to maintain the privacy of your protected health information (PHI), give you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

How We Use and Share Your Health Information

We use and share your health information for:

  • Treatment — coordinating your care with doctors, hospitals, or other providers involved in your care.

  • Payment — billing you, your insurance, or Medicare/Medicaid for services provided.

  • Health Care Operations — internal activities like quality review, training, and business planning.

We may also share limited health information without your written authorization when required by law, such as for public health reporting, health oversight activities, or in response to a court order. Any other use or disclosure requires your written authorization, which you may revoke at any time.

We do not release your health information to any doctor, hospital, or other agency involved in your care without your written consent, except as described above.

Your Rights

You have the right to:

  • Request restrictions on how we use or share your health information

  • Request confidential communications by an alternate method or location

  • Inspect and copy your health record

  • Request an amendment to your health record

  • Receive an accounting of certain disclosures we've made

  • Receive a paper copy of this notice, even if you agreed to receive it electronically

  • File a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated, without fear of retaliation

Our Duties

We are required to:

  • Maintain the privacy and security of your health information

  • Notify you if a breach of your unsecured health information occurs

  • Abide by the terms of this notice

  • Notify you if we change this notice, and post the updated version

Changes to This Notice

We reserve the right to change this notice. Any changes will apply to health information we already have as well as information we receive in the future. The current notice will be posted at our office and on our website.
